A report by the Deputy Health and Disability Commissioner slams Health NZ for systemic failures after an 82-year-old man’s fatal aneurysm was misidentified as a hip sprain.

The Incident: A Fatal Oversight
In June 2020, the patient was taken to Waikato Hospital via ambulance after being found unable to move, pale, clammy, and nauseated. Despite a known medical history of a large abdominal aortic aneurysm (diagnosed a decade prior), a junior doctor (house officer) attributed his symptoms to a musculoskeletal hip injury.
Key findings from the Commissioner's report include:
Misdiagnosis: The junior doctor suspected a hip sprain and discharged the patient.
Lack of Oversight: There was inadequate supervision of the junior doctor by senior consultants during the assessment and discharge process.
Poor Documentation: There were no records of important discussions supposedly held between the junior doctor, the ED registrar, and the consultant.
