The second phase of Dr. SV marks a significant shift from general teleconsultation to specialized chronic disease management. For the 40% of hypertensive Salvadorans who are currently undiagnosed, the AI acts as a frontline screening tool, generating automatic laboratory orders and scheduling follow-ups. If a high-risk patient misses three consecutive appointments, the system triggers a mandatory human follow-up call, ensuring a closed-loop care cycle that was previously impossible in the traditional public health network.

Integration with Google Cloud and Gemini AI

The platform is a centerpiece of El Salvador’s seven-year, $500 million digital transformation agreement with Google. Google Cloud officials confirmed that the system utilizes the Gemini model to assist physicians with real-time diagnostic support. While the AI provides an initial assessment with a reported 93% accuracy rate in trials, President Bukele emphasized that human doctors remain the final decision-makers. The system is currently equipped to handle up to 30,000 appointments daily, significantly expanding the reach of the Hospital Nacional El Salvador.

Streamlined Treatment and Digital Records

A key feature of the Phase 2 rollout is the "Single Digital Health Record," which follows the patient across the entire public health network. Upon completing a virtual consultation, patients receive instant QR codes on their mobile devices. These codes are redeemable for free medications and lab tests at over 400 affiliated pharmacies and laboratories nationwide.
