The primary focus of the review was the "information silo" between the police, Oranga Tamariki, and Corrections. Despite Boulter’s history—which included making nearly 600 harassing calls to the victim in the two weeks prior to her death—agencies failed to consolidate this data into a unified risk assessment. The Child Protection Investigation Unit, an independent body within Oranga Tamariki, confirmed that the results of the review have been shared with Corrections to help "identify opportunities for improved communication." This follows internal reports from late 2025 which indicated that natural justice processes for affected staff had already commenced.

Echoes of the Malachi Subecz Tragedy

The failure to share information effectively is a recurring "red flag" in the New Zealand social services sector. The 2022 inquiry into the murder of five-year-old Malachi Subecz yielded several recommendations intended to bridge these communication gaps. However, the current findings suggest that the Child Protection Investigation Unit’s own work has been "hampered by the exact same problem" it was designed to fix. Reports from February 2026 indicate that 24 children have been killed by caregivers since the Subecz inquiry, fueling criticism that children are "no safer" today than they were four years ago.

Departmental Accountability and Future Inquiries

The Department of Corrections has also completed its parallel investigation into its interactions with police and Oranga Tamariki regarding Boulter. While the department has identified "cross-agency learnings," it has withheld specific details to avoid compromising potential coronial proceedings or further judicial inquiries. The objective of the finalized work is to create a more collaborative framework that triggers automatic alerts between agencies when a known offender initiates a new relationship or displays stalking behavior.
